Grey’s Anatomy is back and more tense than ever.
In PEOPLE’s exclusive clip of the season 21 midseason return, Jo Wilson (Camilla Luddington) is eager to find out if her pregnancy survived the violent robbery — which left Lucas Adams (Niko Terho) potentially shot — in the fall finale. As Miranda Bailey (Chandra Wilson) gives Jo an ultrasound, she makes more than one discovery.
“You are not your doctor,” she tells Jo, who attempts to conduct her own sonogram.
Bailey goes on to assure Jo that her placenta has no abruptions and, when she finds the heartbeat, the two share a sigh of relief.
“What about the other one?” Jo then asks.
A shocked Bailey does a double take before questioning, “There’s an ‘other one’?”
“Okay, let me see,” she replies to Jo’s unsure nod yes. When Bailey finds the twin, she declares: “Oh, there it is. Oh, beautiful!”
“Just these two, right?” she quips, and Jo laughs, “As far as I’m aware.”
Just as things seem to be calming down, Jo lies back in the hospital bed and feels a sudden pain.
The clip comes to a suspenseful end with Jo crying in agony while Bailey begins moving her to the OB ward.
“Where’s Lincoln?” Bailey shouts amid the chaos, calling for Jo’s partner, Atticus Lincoln (Chris Carmack).
The first half of Grey’s Anatomy season 21 left viewers anxiously wanting answers after Jo and Lucas were trapped inside a convenience store with an armed robber. While being held hostage, Lucas learned that Jo was pregnant when she worried that she was bleeding, and in an attempt to take down the robber, he was potentially shot.
Earlier in the season, Jo was struggling to find balance in her life with two kids at home and two on the way. At one point, she even admitted to Amelia Shepherd (Caterina Scorsone), that she and Link “haven’t been in the best place lately.”
Jo’s fear of loss is nothing new for the Grey Sloan resident, and Amelia offered some words of encouragement after having been in a relationship with Link in the past.
“I would be lying if I said that the years of friendship between the two of you didn’t bother me, but I think it was Link who was lying all along to himself,” she shared. “I think underneath it all, he always wanted you.”
Still, Jo admitted to Link that she was worried he wouldn’t stick around, saying, “What if you leave me? Everyone else has. I don’t want to do this alone.”
Grey’s Anatomy airs Thursdays at 10 p.m. ET on ABC.
